Need a passport or looking to renew? Congresswoman Maggie Goodlander is partnering with the National Passport Center to host Passport Fairs to help New Hampshire residents apply for a new passport or renew an eligible passport by appointment.

On this page, you’ll find everything you need to prepare for your appointment, including event details, how to register, what documents to bring, passport fees, acceptable forms of payment, and answers to frequently asked questions. Be sure to review the information below before your appointment to ensure you have everything needed to complete your passport application.

To apply for a U.S. passport book and/or card, you must bring the following:

Application:

•  A completed passport application form DS-11 if you are applying for the first time or don’t meet requirements for passport renewal (see below*).
•  A completed passport application form DS-82 if you meet the requirements for passport renewal (see below*).

 Citizenship evidence:

• Certified U.S. birth certificate (must have a seal of the issuing office), Naturalization certificate, or previous U.S. passport.
• Photocopy of your citizenship evidence.

 Identification:

•  Driver’s license, DOD Common Access Card, military dependent ID or state ID (if bringing an out of state ID please bring another form of ID with you – work ID, student ID, insurance card).
• Photocopy of your ID – front AND back.                                                                                                                   

If changing your name on your current passport:

•  A certified copy of your marriage certificate or court-ordered name change.

  

Recent passport photo:

•  Must be 2×2, in color, white background, no hats, glasses, no military uniforms or camouflage.                                                                                                                                             

Which form do I use (DS-11 or DS-82)?

Apply with the DS-11 form if you respond “Yes” to at least one of the following statements:

•  I am applying for my first U.S. passport book or card.
•  I am applying with my child who is under age 16. Please note: ALL CHILDREN MUST BE PRESENT AT THE TIME OF APPLICATION SUBMISSION. If both parents cannot be present at the time of application, the non-applying parent can submit a notarized form DS-3053.
•  My most recent U.S. passport was issued when I was under age 16.
•  My most recent U.S. passport was lost, stolen, or damaged.
•  My most recent U.S. passport was issued more than 15 years ago.

Apply with the DS-82 passport form if you have a U.S. passport AND it meets the following requirements:

•  Can be submitted with your application.
•  Is not damaged.
•  Has never been reported lost or stolen.
•  Was issued within the last 15 years.
•  Was issued when you were age 16 or older.
•  Was issued in your current legal name (or you can provide a document like a marriage certificate, court order or divorce decree to show you have changed your name).

Passport Fees (to be paid at the event):

 Application Execution Fee for the DS-11 form (routine service 4-6 weeks processing time):

• $35 – can be paid by  check or money order ONLY (No cash, debit, or credit cards)

AND

Passport fee:

• Adults: $130 – Check OR money order ONLY (no cash, debit, or credit cards)
• Minors: $100 – Check OR money order ONLY (no cash, debit, or credit cards)
• For DS-82 renewals: no execution fee; $130 passport renewal fee – Check OR money order ONLY (no cash, debit, or credit cards)

NOTE: Expedited service is available for an extra $60 plus $23.36 for express mail return of your passport. Expedited processing times are currently 2-3 weeks.
